That is a nice layout isn't it? That was made by leyslie and you can find it at ohflare. I'm not sure if its a members only comm (I'm a member so I can see them anyway) but you'll have to search through the layout tags to find it (and remember they may have edited the settings when they put it to theirs so it might not be exactly the same).
What goes wrong when you add headers? The one you're using now looks fine.
